SJOGCS Person Centred Approach (PCA) Policy Webinar

This 21-minute webinar introduces SJOGCS Person-Centred Approach (PCA) Policy.  

The SJOGCS Person-centred Approach (PCA) Policy outlines how Personal Plans—made up of a Person-centred Plan based on an individual’s Life Vision, and Personalised Care and Support Plans focused on their health, safety and wellbeing—are developed, implemented and reviewed so that each individual supported can live their best life.  

Grounded in five core principles of a Person Centred Approach (self-determination, inclusion, valued social roles, contribution and participation, and a Human Rights Based Approach), the policy requires staff across all services to work collaboratively through the four stages of person-centred planning, ensuring accessible communication, meaningful outcomes and goal setting, all linked to the individual’s Life Vision, and evidence based evaluation guided by frameworks such as the HSE Nine Outcomes, New Directions and HIQA standards.  

Keyworkers, line managers, multidisciplinary teams and leadership each have defined responsibilities in supporting consistent, high quality person-centred practice, underpinned by SJOGCS’s 18 Organisational Commitments, with ongoing audit, review and training ensuring continuous improvement and alignment with the individual’s Life Vision.
